S-tier items
Coffee
Coffee is non-negotiable. Each cup restores roughly five sanity per sip with multiple uses before cooldown. Desk players burn sanity on cursed photos (~10 sanity when picking up some prints), CCTV void zooms, and Head Banger events. Treatment players lose sanity during Shapeshifter struggles.
Buy coffee every intermission until your team consistently finishes above half sanity. Place machines near reception if your layout allows shared sips during queues. Secretary mains should still buy coffee even with class sanity perks — perks reduce pressure, they do not delete it.
Fire extinguisher
Fire extinguisher jumps to S-tier once Patient on Fire or Fire in Room emergencies trigger — commonly around shift 3–4. One extinguisher can serve co-op if dropped centrally; competitive teams stash a second near surgery (Room 8) to avoid wing runs mid-timer.
Some accounts receive a free extinguisher at shift 3 start; buy duplicates if your team fires often or runs Paramedic escorts far from lobby.
A-tier items
Taser
Taser stuns Shapeshifters and some hostile forms after bad admits. Many accounts unlock a free taser after Shift 3; until then, buy one if anomalies slip past the shutter. Security class spawns with taser access — coordinate so four players do not duplicate purchases.
Prefer taser over gun for F2P teams: lower collateral risk in tight wards and usually cheaper upkeep.
Gun
Gun deletes anomalies that melee cannot safely handle. Secret Agent starts with gun charges each shift — skip shop gun if someone runs Agent. Otherwise buy one ~$400 Cash gun for a dedicated hunter, or use Robux bundle near reception on some builds.
Kills may refund small sanity chunks on certain classes at high level. Do not shoot normal patients or teammates — sanity and score penalties hurt more than the monster did.
Anomaly scanner
Scanner (~$150) sends a beam that turns red on patients showing anomaly symptoms, green on clean reads, yellow out of range. Two-minute cooldown means it assists — not replaces — the three-layer anomaly check (window, photo, CCTV).
Secret Agent includes scanner at level 1. F2P desk players buy scanner after coffee + extinguisher if photo/CCTV mismatches still get through.
B-tier items
Mint tea
Mint tea (~$100) restores sanity over time instead of instant coffee bursts. Useful for Psychologist or Doctor between heal cycles, or when Dr. Harlow dialogue hints at tea restocks. Lower priority than coffee until baseline sanity is stable.
Deployable camera
Deployable camera (~$600) places an extra CCTV feed for blind spots in lobby or hallways. Strong for teams weak on controls tablet sweeps — expensive early. Buy after shift 4–6 when Disguised Shapeshifter and camera-only twitching appear.
Cola (Run Fast Cola)
Cola boosts movement speed for ~60 seconds — clutch when critical patients need room assignment under timer. Buy situationally when Ambulance Rush icons appear on the pre-shift briefing.
Chocolate bar
