Classes in Animal Hospital (Anomaly) define your sanity budget, movement speed, starter gear, and how fast you complete treatment interactions. You unlock them in the lobby Class Shop with Animal Coins earned from finished shifts and Journal entries, or with Robux for premium roles. This tier list ranks all eleven current classes for online co-op on released servers — not solo theorycraft.
Every class levels through play (typically up to level 3). Perks scale sanity bonuses, starter money, carry capacity, and combat tools. We rank based on level-1 impact plus max-level ceiling, because most players swap roles before maxing everything.

S-tier breakdown
Surgeon
Surgeon is the most expensive Animal Coin class and the strongest free-to-play anchor for mid and late shifts. Each successful heal refunds sanity and grants a short movement speed boost — longer at level 2 and doubled sanity at level 3. That loop matters when Ambulance Rush sends critical patients across wings while you still need to grab bandages or extinguishers.
Downside: 2,500 coins is a long grind if your team mis-admits anomalies. Learn anomaly tells before buying Surgeon; wasted nights delay the unlock.
Doctor
Doctor sits one tier below Surgeon in cost but remains S-tier for consistent medic play. Passive +1 sanity whenever you heal a patient stacks with bonus starting sanity at higher levels (+15 to +20). Utility stat also nudges interaction speed in treatment rooms.
Ideal co-op role when one player permanently runs medical wings while others hold reception. Pairs well with Head Nurse inventory support.
Secret Agent
Secret Agent is Robux-only and breaks anomaly difficulty spikes. You spawn with a gun (limited shots per shift, scaling with level) and a reusable scanner that highlights suspicious patients during rushes. Extra carry capacity helps haul coffee and tools.
Not mandatory in four-stack co-op if someone runs Security with taser, but unmatched for solo players or teams that admit too many Shapeshifters. At max level, gun kills can refund sanity and cash.
Head Nurse
Head Nurse is premium support: expanded inventory lets one player stock coffee, mint tea, bandages, and extinguishers without constant lobby trips. Sanity perks skew toward staying calm during multi-patient chaos.
Best when your group splits — reception + runner + dual medics — and the Head Nurse becomes the mobile supply cache.
A-tier breakdown
Firefighter
Firefighter (roughly 1,100 Animal Coins, community-compiled) arrived with The Hazmat Team Arrives update (July 2026). The elephant-like silhouette spawns with an auto-recharging blue fire extinguisher in inventory — no shop trip required at shift start.
Level perks (community-tested):
- Sanity immunity to fire floor tiles
- Sanity recovery when extinguishing fires
On slime shifts, the same extinguisher knocks Slimebert back temporarily (community consensus: cannot permanently kill). Firefighter is A-tier baseline, climbing toward S-tier on hazard-heavy metas where Float duty is constant — see Hazmat Slime Shift.
Downside: less heal sustain than Doctor/Surgeon; do not pick Firefighter expecting to solo-carry DNA rooms while also owning desk.
Security
Security (1,250 Animal Coins) starts shifts with taser access before free unlock milestones on some accounts. Strong pick for the player who floats between desk and ward to takedown Shapeshifters. Sanity is average; bring shop coffee.
Levels 2–3 post-Harlow also start with Deployable Camera plus X-Taser — excellent Ambulance nights. F2P alternative to Secret Agent when budget is tight.
Psychologist
Psychologist (500 Animal Coins) offers a higher sanity pool and perks that reduce panic from CCTV zooms or cursed photos. Excellent dedicated reception main on shifts where camera tells dominate.
Less heal utility than Doctor; do not pick Psychologist expecting to carry surgery rooms alone.
Paramedic
Paramedic (250 Animal Coins) emphasizes speed — critical for escorting flagged patients before timers expire. A-tier mobility, B-tier sustain. Upgrade path if your team lacks a runner once Ambulance events become common (often shift 4+).
B-tier breakdown
Secretary
Secretary (120 Animal Coins) is the best first purchase: sanity bonuses at the check-in desk plus improved shift cash earnings. Perks reward players who master controls for shutter, photo, and PC registration without admitting fakes.
Falls off when entire teams outgrow sanity issues, but never bad as a dedicated reception specialist.
Nurse
Nurse (20 Animal Coins) is a stepping-stone — slight inventory and sanity bump over Intern. Buy it early, then save for Doctor or Psychologist. Cheap enough that skipping it saves only one mediocre shift.
C-tier: Intern
Intern is the free default. Bonus sanity per level helps brand-new players learn the loop, but no starter tools, no combat kit, and lower utility than every paid role. Replace Intern after your first few successful nights.
Use Intern shifts to memorize Ratthew baseline checks and photo comparison without risking expensive class unlocks.
