Random events interrupt Animal Hospital Night Shift once the mid-game curve begins. Unlike check-in anomalies — rejected at the Shutter — events spawn inside the clinic while patients wait in treatment rooms. Each event carries its own timer, Sanity cost, and optimal responder. Mis-prioritization kills real patients, drains Sanity team-wide, and loses Ambulance nights when the desk admits mimics because everyone chased fire. This guide catalogs major events, timers, and co-op triage order.

When events start

Timed emergency rotation begins Shift 3 alongside the Supplies Shop, Taser, and Emergency Wing. Ambulance events debut Shift 4 and dominate Shifts 4–5 before endless mode. Shift 1–2 rarely fire the full pool — use early nights to learn treatment without alarm stacking.

Event frequency increases after Shift 5 in endless runs. Expect overlapping alarms: Death Ritual plus patient on fire plus Ambulance horn within the same minute.

Event priority framework

When multiple alarms activate:

  1. Shortest timer first — monster eating patient (~30s) beats critical patient (~120s).
  2. Patient death prevention — fainting and critical vitals before cosmetic hazards.
  3. Desk never empty on Ambulance nights — one player holds Shutter while others sprint.
  4. Sanity recovery pauses only if no patient timer is active.

Ambulance events (Shifts 4–5)

The Ambulance forces multiple patients into the clinic in rapid succession. Horns and UI alerts pull attention from treatment rooms toward intake.

Response plan:

  • Check-in Lead stays at desk — full window, photo, CCTV on every arrival.
  • Float pre-positions between desk and right wing to escort admitted patients.
  • Pause non-critical room tasks only if surgery timer not active.
  • Reject aggressively — one Skinwalker during Ambulance often wipes the lobby.

Patient on fire

Fire events spawn burning tiles or ignite patients in treatment rooms — commonly Rooms 1–6 corridors.

  1. Grab Fire Extinguisher (free beside Supplies Shop from Shift 3).
  2. Sprint to flagged room; extinguish patient and floor tiles.
  3. Resume treatment prompt after flames clear.
  4. Avoid walking through fire — Sanity drains per second.

Fire Extinguisher also counters some ghost and hider encounters — Float should carry one even outside fire events.

Patient fainted

Fainting patients collapse in rooms or hallways with ~60-second revival windows.

  1. Assign nearest player immediately — do not finish coffee sip first.
  2. Interact with patient per on-screen revive prompt.
  3. Return to treatment chain once conscious.

Neglected faints drain Sanity and count toward failure. During Ambulance, call “faint Room 3” before admitting the next visitor.

Death Ritual

Death Ritual events spawn candles, ritual circles, or altar interactions — roughly 40–60 seconds to resolve.

  1. Designate a ritual responder each shift so roles do not overlap.
  2. Extinguish candles or complete interaction sequence before timer expires.
  3. Coffee can interact with some candle mechanics without extra Sanity cost when timed well — see How to Manage Sanity.
  4. Failure spawns heavy Sanity loss or enemy spawns depending on variant.

Swap ritual duty each event — volunteers burn out Sanity fastest.

Critical patient

Critical vitals alarms tie to Room 7 heart monitor workflows or hallway emergencies with longer ~100–120 second budgets.

  1. Imaging or Treatment Lead responds first.
  2. Stabilize monitor rhythm or administer prompted emergency item.
  3. Delay new admits until vitals flatline risk clears.

Critical events overlap well with DNA timers — DNA owner calls handoff before sprinting left wing.

Surgery Monster (Room 8)

A hostile spawn during active Room 8 surgery with ~45-second counter.

  1. Surgeon Class or designated Room 8 player finishes instrument sequence if possible.
  2. Taser or gun if monster interrupts surgery steps.
  3. Never abandon surgery table mid-timer unless second player can swap in seamlessly.

Stacked with Ambulance, defer new surgery admits until current operation completes.

Monster eating patient (Bed Monster)

Bed Monster events give the shortest window (~30 seconds). Black hands drag patients under beds.

  1. Stock maple syrup after Ron from Accounting hints — pour to lure monster away.
  2. Stay out of red under-bed zones while rescuing.
  3. If syrup unavailable, kite and interact repeatedly — higher Sanity cost.

NPC-driven events (not random timers)

Some encounters feel like events but are storyline:

  • Officer Duckman searching for Barney — choose hide versus reveal; affects coffee machine reward.

These do not replace timed emergencies but compete for attention — Float handles NPCs when desk is mid-Ambulance.

Four-player template:

  • Check-in Lead: Shutter + Ambulance intake only during surges.
  • Treatment Lead: DNA rooms + fainting response right wing.

  • Float: Fire, Death Ritual, Bed Monster, Skinwalker, camera repair.

Three-player: Merge Float into Imaging until Shift 7.

Two-player: Alternate event call — “I fire, you desk” — before horn sounds.

Event failure recovery

If a timer expires:

  • Assess Sanity team-wide — pause admits if below fifty percent.
  • Tighten Shutter — anomalies exploit chaos after failed events.
  • Restock extinguisher and syrup between shifts, not mid-alarm.

Endless mode event stacking

Shift 6+ may fire two timed events plus Ambulance residue patients. Voice comms matter: “ritual first, fire second, faint Room 2.” Typing loses seconds.

Hazmat slime special shift (July 2026+)

The Hazmat Team Arrives update added a secret slime shift variant (community-compiled) that behaves differently from standard timed emergencies. The hospital floods with green slime, Slimebert roams between rooms, and fire extinguisher bursts knock the entity back without a permanent kill.

How slime shifts interact with normal events:

  • Standard patient on fire timers can still trigger — Firefighter class (1,100 Animal Coins) or shop extinguishers cover both fire tiles and Slimebert knockback
  • Ambulance surges during slime are especially dangerous — Check-in Lead must hold desk while Float owns extinguisher duty
  • Stamping the containment form summons Hazmat NPCs to scrub main halls — not a substitute for clearing patient queues
