Not every face at Animal Hospital (Anomaly) is a random patient generator. Named characters script tutorials, shop interactions, comic relief, and — critically — baseline looks that anomalies mimic on photo or CCTV. Knowing who is “normal” makes anomaly checks faster for co-op reception mains.
This guide covers the core NPCs players search most: Dr. Harlow, Barney, Liz, Sam, Ratthew, and Ron from Accounting. Other background staff (shopkeeper nurse, Officer Duckman) appear in shifts but these six anchor most community guides and Journal character pages.
Journal character pages
The lobby Journal (below Classes, Hazmat update) tracks character entries alongside anomalies and achievements. Meeting or interacting with named NPCs during live shifts unlocks pages that pay Animal Coins. Character pages are usually safer to farm than anomaly admit-first pages — prioritize Harlow, Barney, Liz, Sam, Ratthew, and Ron lines before gambling Tiny Anomaly admits. Dr. Harlow
Dr. Harlow is the senior clinician framing your night — pre-shift briefings, difficulty warnings, and lobby presence near the Supplies Shop. Harlow dialogue often hints at upcoming emergencies (fires, ambulance rushes) or new stock like mint tea and anomaly scanners after updates.
Treat Harlow as your meta narrator, not a patient to admit. Interactions happen between shifts or during scripted beats, not through the check-in shutter. When patch notes mention “Harlow’s Favorite Update,” expect shop rotation changes — revisit the items tier list for new buy priorities.
Co-op tip: whoever reads Harlow’s warning aloud saves thirty seconds of menu confusion before ready-up.
Barney
Barney is a special anomaly-class NPC who usually does not request normal hospital admission. Instead he asks for favors — most famously coffee — at or near the desk. Helping Barney often rewards utility (community reports include bonus coffee machine access or sanity-friendly outcomes) whereas hostile handling drains sanity.
Barney differs from queue patients:
- No standard stamp-and-badge flow
- Interaction is social / item handoff
- Mistreating him wastes resources better spent on real queues
Do not confuse Barney with Head Banger — another coffee-related desk pest. Head Banger smashes glass with hollow eyes; Barney is conversational weirdness. Both tie into coffee economy — stock cups from the Supplies Shop before long nights.
Liz
Liz appears as supporting clinic staff in community rosters — often near reception or treatment wings depending on shift scripting. She reinforces co-op atmosphere more than hard mechanics, but recognizing Liz matters when mimic anomalies swap faces: if Liz-model NPCs only appear in scripted scenes, a “Liz” at the patient window without context should trigger full three-layer checks.
Voice lines and ambient dialogue from Liz sometimes coincide with sanity pressure — lower volume if your team calls photo mismatches over comms.
Sam
Sam is a Hazmat-affiliated NPC introduced with The Hazmat Team Arrives update (July 2026, community-compiled). Sam appears during slime shift sequences when the Hazmat team arrives to scrub corridors, and can be hired permanently in the lobby afterward for roughly $250 cash (community-compiled).
Once hired, Sam persists across shifts and assists with:
- Slime tile cleanup on special nights
- Secondary anomaly containment tasks (community reports vary by script)
- Reducing Float workload when Slimebert and standard fire events overlap

Ratthew

- Mimic Patient anomalies copy Ratthew’s ears, markings, or posture on print/CCTV while the window shows a different animal
- Co-op teams call “Ratthew baseline” when asking if ears match prior nights

Ratthew is not immune to anomalies pretending to be him. The real Ratthew and a mimic both require checks; the difference is tells, not name tags.
Ron from Accounting
Ron from Accounting adds humor and paperwork flavor — an office rat tangled in hospital bureaucracy. Community lists place him among recurring character rosters rather than combat enemies. Expect Ron in lobby or dialogue beats about billing, shift reports, or absurd admin tasks.
